Non Traumatic Stable Osteocondritis (Grade1) Lesion of Talus: A Case Report and Physiotherapy Management

Abstract: Introduction Osteochondritis dissecans (OCD) is an acquired idiopathic lesion of subchondral bone that can produce delamination and sequestration with or without articular cartilage involvement and instability. Ankel is the third most common location after the knee and elbow. The lesion usally effect talar dome of ankle. Here reporting a case of osteocondritis of medial talar dome in a 44 years old female without any history of trauma. OBJECTIVE Study done to report the effect of low level laser therapy and active assisted exercise program in osteocondral lesion of Talus (grade1, stable). PHYSICAL EXAMINATION Physical examination done by measuring ROM, strength, palpation of part and pain assessment recorded (VAS). Diagnosis was made with MRI of left ankle. INTERVENTION Patient treated conservatively as patient has OCD grade1 stable so Immobilization with placement of a air cast boot for left ankel a total of 6weeks, non-weight-bearing on walker and patient receive low level laser therapy and active assisted exercise program for left ankle. Outcome After 12 weeks patient ROM was complete pain free with muscle strength of 4/5 left ankle. Walking (short distance) was pain free. Repeat MRI at that fourth month follow-up visit showed Lesion significantly less cystic in appearance compared to previous study. Discussion Physiotherapy management provide great relief in symptoms by various modalities and methods during conservative treatment, Modalities like laser help to promote healing. Exercises help to achieve maximum recovery, improve strength, improve balance and propriocepction.
