Penile Fracture: Incision Over the Defect Approach

Abstract: Objective: This study was done to evaluate the safety and efficacy of the incision over the defect approach in the management of penile fracture. Material and methods: This prospective observational study, conducted from January 2022 to June 2023, included a total of 37 patients of penile fracture diagnosed by history, examination and Ultrasonographic localisation of the defect. Surgical exploration and repair was done within 6 hours of presentation. Results: All patients underwent surgical exploration and repair under spinal anaesthesia and incision over the defect approach was used in all patients with 2 patients requiring conversion to conventional degloving technique due to failure of locating the defect. All the patients had Corpus cavernosus involvement only and none had a urethral injury.1 (2.7%) patient had a bilateral cavernosal injury. The mean operative time was 42.2 minutes.2 (5.40%) patients had surgical site infection on follow up. Conclusion: Quick diagnosis and surgical repair of penile fracture is essential and the incision over the defect approach is a simple, safe and efficacious procedure with minimal complications.
