Abstract: Background: Type 2 Diabetes Mellitus (T2DM) is characterized by hyperglycemia due to insulin resistance and impaired ? - cell function. The relationship between fasting serum insulin levels and fasting blood glucose (FBG) is pivotal for understanding disease pathophysiology. Objectives: In this study, we aimed to analyze the association between fasting serum insulin levels and fasting blood glucose in patients diagnosed with T2DM. Materials and Methods: A hospital - based cross - sectional study included on 50 T2DM patients aged 20 - 65 years. Fasting serum insulin and blood glucose levels were measured after overnight fasting. Insulin resistance was calculated using the HOMA - IR index and correlation analysis were performed. Results: Fasting serum insulin was significantly correlated with HOMA - IR (p = 0.03), indicating its role as a surrogate marker of insulin resistance. However, no significant association was found between fasting blood glucose and fasting serum insulin in this cohort. Conclusion: The findings highlight the complexity of insulin - glucose dynamics in T2DM and underscore the importance of targeting insulin sensitivity to manage glycemic dysregulation effectively.
